Gaston Family Health Services Online Enrollment

Did you know that your child can begin receiving behavioral health services at school?  Southwest is proudly partnering with Gaston Family Health Services to provide school based interventions to any child who may struggle with academics, developmental issues, relationships, anxiety, depression, substance abuse, behavioral concerns, grief, transition, and more.  These services are provided to any child regardless of insurance type or ability to pay.  No service will ever be denied.  Signing up is now easier than ever.  Simply fill out an enrollment packet or click the provided link below and you will be redirected to a secure enrollment form.

Hickory Shared Access
